Initial Comments: The purpose of the inspection was to conduct complaint #00161762 investigation on 3/23/2026. A full inspection was not conducted at this time. A copy of the Notice of Inspection Rights was provided at the time of the inspection. Ratios observed were: Infants - 1:3 1s - 1:5 2s-3s - 1:7, 2:10 3s - 1:9 4s-5s - 1:13 There were 4 staff members interviewed during this investigation. The Compliance Officer contacted the complainant via phone on 3/11/2026. Upon completion of the complaint investigation, it was determined, based on observation and interview, that 1 of 4 allegations was substantiated. The other 3 of 4 allegations were unable to be substantiated due to a lack of sufficient evidence. The following deficiencies were observed and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The following was discussed, but not limited to: *Rosters to reflect the correct number of children in the classrooms. *Non-enrolled children must not be mixed with enrolled children. *A hand-washing sink must be next to the diaper changing surface for staff to use when changing diapers and washing an enrolled child during or after diapering.
INSP-0165055 2025-12-19 Initial Monitoring Complete
Initial Comments: The following deficiencies were observed at the Initial Monitoring Inspection conducted on 12/19/2025, and are subject to changes pending programmatic review. Please submit the Plan of corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 3 of 3 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following items were discussed, but not limited to: 1. Infant room capacity is limited to 5 until a modification application is requested. 2. Staff files should reflect documentation and the new date after the change of ownership occurred. 3. Ensure all staff, including owners, directors, and salaried employees are signing in and out daily.
INSP-0160873 2025-10-08 Compliance (Initial) Complete
Initial Comments: The following deficiencies were observed at the Initial Inspection conducted on 10/8/2025, and are subject to changes pending programmatic review. Please submit the Plan of corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 13 of 13 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following was discussed but not limited to: 1) Revised architect plan to include the 4th sanitary unit, 2) Staff files will be updated to include the following: Date of employment with new licensee, TB test attestation, DPS fingerprint clearance card verification, new staff training completion, DCS background checks, Criminal history affidavit, 3) Tummy time rules, 4) Verifying Staff #1’s GED program completion, 5) Per rule, staff must have a valid fingerprint clearance card within 30 calendar days of 18th birthday, 6) Cleaning of ceiling fans, rugs, and bathroom stools.
