Findings: After a review of 3 staff files, it was determined the following were working in the capacity of a teacher-caregiver, but were lacking documentation to qualify as a teacher-caregiver: Staff #2 (start date 9/7/2021) had only one month of the required 6 months of previous child care experience. Staff #2 was observed taking enrolled children alone to the bathroom during the inspection. Staff #3 (start date 9/27/2021) is currently enrolled in high school and had no previous child care experience. Staff #3 was left alone outside on the playground with a group of enrolled children during the inspection. Staff #1 (start date 6/17/2021) was the only teacher-caregiver qualified staff member on site. All three staff members were observed in the classroom with 25 enrolled children upon arrival, but then Staff #2 & Staff #3 took a smaller group of children outside and Staff #1 stayed in the classroom with the remaining children.

Findings: Upon arrival at the facility/program, the front office was closed and there was no access to the classroom where the program was operating. The Surveyor had to call the program number, but could not get through. Then the Surveyor called the regional director and made contact and then was finally let into the school and led to the classroom where the program was operating. The Surveyor did not have immediate access, as required.
