Violation Observed: The center will need to submit a NJCCIS Space Request and Capacity Decrease to the Office of Licensing as they installed a bathroom in room 10 taking away classroom space. 6-13-23: The center moved the wall in room 3 to include a hallway bathroom and this space needs to be remeasured as well.

Violation Observed: Ensure that all cribs meet CPSC standards and that the CPSC documentation is maintained on file at the center. The center replaced pack n play pads with portable crib mattresses. The center needs to either provide documentation that the newly installed mattresses are for pack n plays, including documentation of the model and make of the pack n plays they are intended for, or cease the use of these new mattresses. 5-4-23: The center needs to cease the use of the new portable crib mattresses and provide properly fitting pads in the pack and plays that are intended for use according to manufacturers instructions. 6-13-23: The center removed all additional mattresses from the pack n plays throughout the center.

Violation Observed: Ensure that all cribs and playpens are equipped with all of the required components, including a firm, waterproof, snugly fitting mattress. Cease the use of the newly installed Playmat mattresses that are not waterproof.

Violation Observed: Ensure that staff in room 7 are either outside with all children or in the classroom with all children. There were 2 children inside the classroom and 8 outside not allowing staff to properly supervise.
