Violation Observed: Ensure that the center adheres to all requirements to prevent comingling in multi-use buildings. A girls school is utilizing 2 rooms and part of the hallway on the second floor of the center, rooms 223 and 224. The center needs to ensure there is no co-mingling between the childcare center and girls school.

Space and room requirements are as follows: all space and rooms within the center to be used by children shall be inspected and approved by the Office of Licensing prior to their use. In making its determination, the Office of Licensing shall consider whether the space is too far removed, remote, or isolated from other areas of the center to be used by children. rooms or areas of the center that are not Office of Licensing-approved for use by children shall be made inaccessible to children.

Violation Observed: Ensure that unapproved space is inaccessible to the children at all times. The center is utilizing room 118, 217, 219, 220, and 221(playroom) which are not on the center's license. The center will need to complete a space request and capacity increase in NJCCIS.

Violation Observed: Ensure that materials are accessible to the children at all times, except during lunch and nap time in all classrooms throughout the center. Classrooms have supplies up high in cabinets. Only one supply at a time was being provided to children to play with. 4-18-23: Rooms 220 and 219.

Violation Observed: Ensure that classrooms that care for children under 18 months of age provide at least four different activity areas and that each area contains at least four types of supplies and equipment. In room 110, there were not enough supplies.

Violation Observed: Ensure that all materials used by the children are age appropriate, non-toxic and safe for use by children. The center has magnetic tiles in several classrooms for children under the age of 3 years old when this toy is manufactured for children ages 3 and up.

For early childhood programs, the center shall provide opportunities for daily rest and sleep as follows: for children from 13 to 18 months of age, the center shall provide daily rest and sleep according to the child's physical needs and ensure that blankets or pillows do not cover the child's face.

Violation Observed: Ensure that blankets do not cover the children's faces while sleeping. In rooms 125 and 126, there were children with a blanket over their head and covering their face. Staff removed at the time of inspection.

When sheets, blankets and other coverings are not stored with sleeping equipment, each of these items shall be stored separately for each child.

Violation Observed: Ensure that each child's bedding is stored separately. In room 212 the children's cots were stacked on top of each other and the sheets and blankets were touching.

The center shall develop and implement an Office of Licensing-approved method to keep track of the location and ensure the safety of all children at all times when under the center’s supervision, including the transfer of supervision from and to parents during arrival and departure and the utilization of off-site locations, including playgrounds and field trips.

Violation Observed: Ensure that adequate procedures for tracking children are adhered to at all times throughout the center. In room 107 staff stated they had 6 children but 7 children were present and then upon return staff stated they had 11 children but 10 were present. In room 125, staff stated they had 14 children but 15 children were present.4-18-23: Room 125, staff stated that they had 16 children but 17 children were present. 4-18-23: Room 127, staff stated they had 8 children but 7 children were present.

Play equipment, materials, and furniture for indoor and outdoor use shall be of sturdy and safe construction, non-toxic, free of hazards, and used in accordance with the manufacturer’s instructions. The center may not use play equipment intended for outdoor use indoors.

Violation Observed: Ensure that all indoor and outdoor equipment is sturdy, safe, free of hazards and used in accordance with the manufacturer's instructions. In room 110, staff need to ensure that infants are fully strapped into swing seats. There was an infant in as swing seat with only a waist buckle on and not the top straps as required.

For early childhood programs, the following shall apply: in addition to the items specified in N.J.A.C. 3A:52-7.7(a)3, the following equipment items or surfaces shall be washed and disinfected after each use, diapering surfaces.

Violation Observed: Ensure that the center washes and disinfects diapering surfaces after each use. In room 110 staff did not clean the diapering surface and only changed the paper between changes. In room 127, staff used a disinfectant as step 1 and soap and water as step 2. They need to ensure the surface it cleaned with soap and water first then a disinfectant.
