Violation Observed: Ensure that all cribs and playpens are equipped with all of the required components, including top rails that are at least 19 inches above the mattress. In room 14, the pack and plays had an insert raising the level of the mattress pad which was above 19 inches.

Violation Observed: Ensure that children in swing seats are strapped in using the buckles. In room 14, there was a child in the swing not buckled.

A classroom staff called Hatzolah, after a child was placed on the floor in the infant room in a sitting position and fell backwards. Per the center administration, the child did not receive any injuries however the center did not notify OOL after the incident.

Violation Observed: Ensure that sleeping equipment is free of pillows, soft bedding, bumpers and other hazards when occupied by a sleeping child. In room 11, the pads were too small for the pack and plays, leaving room for the child to lay on the vinyl, with no pads or sheets.
