Violation Observed: Ensure that adequate supervision is maintained at all times throughout the center. There were children from room 3 unsupervised in room 2 during drop off and after breakfast when the class returned to room 3 and left 3 children still eating at the table. 6-6-23: Ensure that children are supervised at all times. At the time of this inspectors arrival, 4 children were observed in the exterior hallway outside room 4 knocking on the door to be let in for the bathroom.

The center shall develop and implement an Office of Licensing-approved method to keep track of the location and ensure the safety of all children at all times when under the center’s supervision, including the transfer of supervision from and to parents during arrival and departure and the utilization of off-site locations, including playgrounds and field trips.

Violation Observed: Ensure that adequate procedures for tracking children are adhered to at all times throughout the center. Staff in room 3, stated they had 20 children when 19 were present. Staff in room 4, stated they had 12 children when 14 were present. Staff in room 1 had to count the number of children present.
