All walls and ceilings including doors and windows were not kept clean and in good repair. The walls in classrooms 2, 3, 4, 5, and 10 were soiled, had peeling paint and were not kept clean and in good repair.

A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space 6, one medication permission to administer giving standing orders for a prescribed eczema cream was not available.

In space 6, one permission to administer form providing standing orders to administer A&D ointment had expired on 3/12/24 and in space 7, one permission to administer form providing standing orders to administer an Epi-Pen had expired on 2/1/24.

Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Six staff members had health information stored in their personnel files.

The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Two staff members with hire dates of 8/1/23 and 9/11/23 did not have documentation of completing Recognizing and Responding to Suspicions of Child Maltreatment on file.
