Findings: LS observed missing emergency forms for 2 of the 3 siblings enrolled. The 1 form that the provider found is outdated. Provider states she has all 3 current emergency forms but she has just misplaced them. The provider states the children are her godchildren and the children of one of her approved substitutes. Provider is to locate emergency forms or have parent complete 3 new ones. Provider is to documentation of correction to OCC ASAP.

Findings: LS observed the addition room that is also approved for child care currently being used as storage with storage bags, extra supplies and stored beverages/bottles. Provider states due to the room not having AC and it being too hot, she is not currently using it for child care and has some seasonal items stored. LS let the provider know that the room is not to be used until it is cleared out. Provider to send a picture as proof that the room is ready to be used again for child care.

Findings: LS observed the addition to the home setup for child care. This room was approved by fire and was inspected initially by LS. However, it was never added to the registration since there was no insulation and the temperature was too cold. The provider has sent added insulation and set the room up for child care. The provider states she just reopened for care this morning after being closed and the children were in the room. LS explained that rooms used for care must be added to the registration prior to use. LS inspected the room and will add to the registration for supervisor approval. Provider states she did not know and will do what is required moving forward. Corrected on site.
