| Initial Comments: There were no deficiencies observed at the time of the Initial inspection conducted on 12/18/2025, and is subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was provided to the Licensee during the inspection. One staff file was reviewed, and the fingerprint clearance card was verified as valid via the DPS website. During the exit interview, the following information was discussed but not limited to: *The fall zones in the outdoor activity area will have a minimum of 6 inches of wood chips *The outdoor activity area will be free from hazards *The lesson plan, daily schedule, and capacity will be posted in the activity room *The posting area will include the requirements as required in R9-5-715.A *Ensure documentation of the 10-day new employee training and current CPR card are included in the staff file *Ensure a second staff is present on the premises when six or more enrolled children are present (R9-5-726.C) | |||
