| Initial Comments: The following deficiencies were observed at the time of the Initial Inspection conducted on 4/20/2026 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the provider at the time of inspection. The fingerprint clearance cards for 1 of 1 staff members were verified to be valid through the DPS website at the time of the inspection. The following was discussed but not limited to: 1). Ensuring that staff files are updated for transferred staff members. 2). Ensuring that hazards are not accessible to enrolled children. 3), Ensuring that the first aid kit has gauze rolls. 4) Ensuring that the capacity is posted in the room. 5). Ensuring that injuries are logged when medical attention is provided. 6) Ensuring that trash cans with food waste have a tight-fitting lid. 7). Ensuring the Emergency, Information, and Immunization cards are updated with the parents/guardians' email address. 8). Ensuring that staff have 24 hours of annual training. 9). Ensuring that parents/guardians sign in and out their child with their signature or a unique identifier. 10). Ensuring that cracked or peeling caulking is fixed. | |||
