Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. *Please submit a copy of the current State Fire Marshal fire inspection when obtained. The following was discussed, but not limited to: Staff #3's fingerprint clearance card will expire on 5/5/2024. There were 3 staff files reviewed. 3 of the 3 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment link was sent to the Director at the time of the inspection.
