Moon Valley High School is located in the northwest area of Phoenix. Our student population of approximately 1700 comes mainly from middle class homes drawing from professional, high technology, and general service and construction trades. Moon Valley is a comprehensive four-year public high school with a strong college preparatory curriculum, business partnerships and extensive general education programs. Accelerated courses are offered in mathematics, English, social studies, science, and foreign language. Glendale Community College is located five miles from our campus and provides concurrent enrollment for qualified students in qualifying programs. In addition, Gateway Community College offers dual enrollment credit for many of our AP courses.

Initial Comments: The following deficiencies were observed at the time of the Annual Compliance Inspection conducted on 9/23/2025 and are subject to changes pending pragmatic review. The Plan of Corrections will not be accepted at this time. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The Emergency Disaster Contact Form was completed at the time of the inspection. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but not limited to: *Discussion of new rule set and where to find the new documents that are needed.

Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact Form was completed at the time of the inspection. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. The Compliance Officer is Sherri Pavlisick.

Initial Comments: There were no deficiencies observed at the time of the Compliance Inspection conducted on 9/26/2023. The Emergency Disaster Contact form was completed at the time of the inspection. The Empower Self-Evaluation was completed at the time of the inspection. The DES Contract Child Care Group Size was in compliance at the time of the inspection. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *Regularly check the temperature of the refrigerator and freezer. *The light brown child picnic table outside will need to be sanded and varnish again soon. The Compliance Officer is Sherri Pavlisick.
