Noncompliance Area: Staff person #4 was unable to name the children in their primary care group.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: It was observed at the time of the inspection that there was not a written policy on file to address potential signs/symptoms of abusive head trauma, strategies for coping with a crying, fussing, or distraught child, nor the prevention and identification of child maltreatment. It was observed that the trash can used by the program did not have a lid.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: Staff persons #1, #2 and #3 do not have documentation of emergency plan training on file and all are outside of their 90-day provisional hire period.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: It was observed that staff person #4 was working with children and the individual record was not available.

Correction Required: An individual record is required for each facility person.

Description: Health assessment, TB test

Noncompliance Area: It was observed that the health assessment of staff person #1 is dated 9/25/19 and does not have record of a subsequent health assessment 24 months later on file. It was observed that the health assessment of staff person #3 is dated 11/3/00 and does not have record of a subsequent health assessment 24 months later on file.

Correction Required: A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Description: Age and Training

Noncompliance Area: It was observed that staff person #1 had 6 of 12 needed training hours on file between 12/19/23- 12/19/24. It was observed that staff person #3 had 9 of 12 needed training hours on file between 12/19/23- 12/19/24.

Correction Required: A staff person shall obtain an annual minimum of 12 clock hours of child care training.

Description: Fire safety - 1 yr.

Noncompliance Area: There was not record of fire safety training for staff persons #1 and #3 on file.

Correction Required: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: It was observed at the time of inspection child #3 left the classroom and went out into the hallway to the bathroom and staff did not see him leave the room. Staff present at the facility did not know he left the classroom until the Certification Representative informed them that he had left the classroom.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: It was observed at the time of inspection child #1's albuterol inhaler did not have a label identifying the child for whom the medication is intended.

Correction Required: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container.

Noncompliance Area: It was observed at the time of inspection child #2 did not have a medication log for their prescription Albuterol inhaler.

Correction Required: An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered.

Noncompliance Area: It was observed at the time of inspection that the facility last completed an emergency drill on 3-24-23.

Correction Required: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility.

Noncompliance Area: It was observed at the time of inspection staff #1s last documented pediatric first aid and cpr training was completed on 10/11/21

Correction Required: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification.

Noncompliance Area: It was observed at the time of inspection that staff #'2's most recent evaluation was completed on 9-13-24 and the most recent evaluation prior to that was completed on 9-18-21 not every 12 months as required.

Correction Required: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months.

Noncompliance Area: It was observed at the time of inspection the facilities emergency plan did not provide for accommodations for shelter of children during an emergency including lock-down. The facility did not have an emergency plan that provides for accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions.

Correction Required: The facility shall have an emergency plan that provides for accommodations for shelter of children during an emergency including lock-down, shelter in place at the facility and shelter at locations away from the facility premises. The facility shall have an emergency plan that provides for accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions.

Description: Letter to parents

Noncompliance Area: It was observed at the time of inspection the letter to parents explaining emergency procedures did not provide for accommodations for shelter of children during an emergency including lock-down and accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions.

Correction Required: The operator shall provide to the parent of each enrolled child a letter explaining the emergency procedures. The operator shall also provide to the parent of each enrolled child a letter explaining any subsequent update to the plan.

Noncompliance Area: It was observed at the time of inspection child #1's emergency contact information was last updated on 2-5-22.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: Approximately three weeks ago child #2 jumped on child#1's back while sitting at the cafeteria tables. An incident report was not provided to child #1's parents.

Correction Required: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file.

Description: Medication log

Noncompliance Area: It was observed at the time of inspection one medication log was completed for all three of the child's medication. (EpiPen, benadryl and albuterol inhaler).

Correction Required: An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered.

Description: Refrigerator

Noncompliance Area: It was observed at the time of inspection that the refrigerator used by the school-age program did not have a thermometer.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator.
