|Findings: After a review of 2 staff files, it was determined the following was lacking documentation of the DPS contact to verify the status of the clearance card: Staff 1 (start date 3/27/2017) had a fingerprint clearance card which was issued on 9/17/2016; prior to employment at the facility.|
|Findings: Staff 1 was the designed person to act on behalf of the director. Staff 1 did not meet the required qualifications.|
|Findings: Staff 1 was observed unsupervised with 16 enrolled children. Staff 1 does not meet the qualifications of a teacher-caregiver.|
|Findings: Upon arrival at the facility, the school office was closed and the gates leading to the classroom were locked. The Surveyor called the phone number on record for the facility, but it went unanswered. The Surveyor called the off-site program coordinator to inquire how to gain entry to the program. The program coordinator called the director designee, who walked all the enrolled children to the gate to let the Surveyor into the school. The staff member stated the gate is locked until 2:30-3:00pm on early release days and parents must call or text to gain entry into the program.|
|Findings: After a review of 2 staff files, it was determined the following was lacking: Staff 2 (start date 11/30/2016) had documentation of freedom from infectious active tuberculosis dated 1/10/2017; outside the required time frame.|
|Findings: After a review of 10 children's Emergency, Information and Immunization Record cards, it was determined the following one was lacking: Child 1: Health care provider's name and phone number..|
|Findings: After a review of 10 Record cards, it was determined the following Record cards were lacking a copy of the child's immunization record: Child 2 Child 3|
|Findings: A current fire inspection was not available at the time of the inspection. The fire inspection available on facility premises from the State Fire Marshall which had various violations and no documentation of repairs or corrections.|
|Findings: After a review of 2 staff files, it was determined the following one file was lacking: Staff 2 (start date 11/30/2016) had documentation of 7 months of the required 2 years of child care experience and 7.5 of the required 72 hours of training to qualify as the director.|
|Findings: After a review of 2 staff files, it was determined the following one file was lacking: Staff 2 (start date 3/27/2017) was listed as the director's designee. Staff 1's file lacked documentation of 1 year of child care experience.|
