Staff Person #1 was terminated from Healthy Kids Extended Day Program on November 6th after Staff Person #1 told me on November 4th that they had altered the date of the Health Assessment which had been originally dated December 17, 2021 and that he had falsified it to make it appear that it was dated December 17, 2024. During November 4-6, I conducted a thorough investigation and found that Staff Person #1's action is a direct violation of Healthy Kids policies, ethics and commitment to being trustworthy and truthful. They admitted falsification of licensing documents gave me the justification to terminate Staff Person #1 from employment at Healthy Kids Extended Day Program on November 6, which I did. |
