| Initial Comments: The following deficiencies were observed at the time of the Initial Compliance Inspection conducted on 10/22/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the provider at the time of inspection. The fingerprint clearance cards for 39 of 39 staff members were verified to be valid through the DPS website at the time of the inspection. The following was discussed but not limited to: 1). Ensuring that the fire extinguishers are inspected annually. 2). Ensuring that feeding instructions are reviewed and updated by parents/guardians frequently. 3). Ensuring that staff that come to the facility to teach an activity have a valid fingerprint clearance card. 4). Ensuring that outside play material that are worn are replaced/fixed. 5). Ensuring that possible hazards on the outdoor activity areas are supervised by staff when children are present. 6). Ensuring that background checks are completed prior to the start of employment. | |||
