Noncompliance Area: Review of files reveal that Child # 1 last health assessment occurred in August 2018. Child # 3,4, and #5 were missing immunizations on their medical verification during the time of the renewal.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. A health report shall include a review of the child's immunized status according to recommendations of the ACIP.

Noncompliance Area: Review of files reveal that Child # 1 and Child #2 did not have their parent's work address and or work number listed on their Emergency Contact form at the time of the renewal inspection.

Correction Required: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: Review of files reveal that Child # 1,3, 4 and 5 did not have the required parental signature notated on their Emergency Contact form at the time of the renewal inspection. Instead, the children had initials placed where a parental signature was requested and there was no way to verify the authenticity of the parental signature at the time of the renewal inspection.

Correction Required: A child's record must contain signed parental consent for emergency medical care for the child.
