Noncompliance Area: Observed staff cubbies in the Bunnies (infants') classroom with plastic bags that were accessible to the mobile infants and young toddlers in care. *Corrected at the time of the inspection. Observed plastic bags on the floor next to the sink in the bathroom area of the Penguins (older toddler) classroom which were accessible to children while using the bathroom. *Corrected at the time of the inspection.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: Observed a sensory table in the Koalas classroom which was missing a wheel causing the table to be unstable. *Corrected at the time of the inspection. Observed the grey upholstered chairs in the Frogs 1 classroom had tears at the top corners of the attached back cushions. Observed the grey upholstered chairs in the Frogs 2 classroom had tears at the top corners of the attached back cushions. The couch in the Frogs 2 classroom had a tear in the right side of the back cushion. The rocking chair in the Frogs 2 classroom had a tear on the side of the chair.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: Observed adult scissors and a utility knife on the teacher's desk in the Hedgehogs classroom which were accessible to preschool age children in care *Corrected at the time of the inspection. Observed a Cricut machine in the Polar Bears classroom which was accessible to preschool age children in care. Observed debris and food crumbs on the trash can next to the sink and step stool located in the Penguins classroom (older toddlers). *Corrected at the time of the inspection. Observed a light in the left closet located in the Frogs 1 classroom which was sitting on top of the metal ceiling tile grid with a loose ceiling tile resting on top of the light. Observed visible dirt and dust along the baseboard trim on the wall in the Frogs 1 classroom. Observed the baseboard strip on the wall by the sink in the Chinchilla's classroom was pulling away from the wall.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Observed chipping paint on the chalkboard ledge and door frame of the closet in the Koalas classroom (preschool), at the closet door (with the mirror) in the Ducklings classroom (infants), at the closet door in the Penguins classroom (older toddlers), behind a bookshelf in the Raccoons classroom (older toddlers) on the window ledge in the Bunnies classroom (infants) at the closet door (with the mirror) in the Hedgehogs classroom (Preschool) and on the down spout on the side of the building which is in the toddler playground.

Noncompliance Area: Observed a paper towel dispenser in the Boys bathroom which was loose. Observed rusting sink fixtures under the sink in the Girl's bathroom. Observed the sink and sink fixtures located in the Pandas classroom were rusted and flaking. Observed the bathroom used by children in the Pandas classroom was not clean and had urine at the base of the toilet. Observed the paper towel dispenser in the bathroom in the Pandas classroom was not secured to the wall.

Correction Required: Toilet areas and fixtures shall be cleaned daily and be in good repair.

Noncompliance Area: Observed prescription medications in the "go-bag" which were not locked or in an area that is out of reach of children. The "go-bags" were on the picnic tables under the pavilion in the pool area while the group was swimming.

Noncompliance Area: The emergency contact information for child #1 and #2 did not include signed parental consent for swimming. Child #1 and #2 were observed in the pool at the time of the inspection. *Corrected at the time of the inspection.

Description: Supervised at all times

Noncompliance Area: Observed staff person #4 was sitting at a table next to the door in the porcupine classroom while 4 younger school age children were playing on the opposite side of the classroom behind a shelf. Staff person #4 was unable to see, direct and assess the children from their position on the other side of the room. Observed staff person #5 was unable to see all of the children during naptime in the Dolphins classroom. Staff person #5 was sitting at a table in the middle of the room, due to placement of the cots behind/next to shelves, 2 napping preschool age children were unable to been seen from where staff person #5 was sitting.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: The pediatric first aid and CPR for staff person #1, #2, #3 was not renewed on or before the expiration of the previous certification. The current training on file was dated 04/13/2024 with the previous training dated 03/24/2022 with an expiration date of 03/2024.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification.

Noncompliance Area: During naptime, observed 1 staff person caring for a group of 12 older toddlers. 11 older toddlers were resting quietly or sleeping on their cots and 1 older toddler was awake and sitting on the other side of the room sliding around on the floor. When a child is awake and no longer resting on their mat, naptime ratios no longer apply.

Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30. The staff:child ratio while toddlers and preschoolers are napping is one staff person to 10 young toddlers, one staff person to 12 older toddlers, one staff person to 20 preschool children.

Noncompliance Area: Observed staff person #1 in the Panda classroom assisting a child in the bathroom. From their position in the bathroom, staff person #1 was able to supervise 3 children napping nearby but was unable to supervise the remaining 6 children napping in the classroom. 1 child was behind a shelf, and 5 children were on the other side of the room which were blocked from view by the wall in the center of the room. There were 4 older toddlers and 6 preschool age children with 1 staff person in the classroom during naptime.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: **During naptime in the Penguins classroom, there was a group of 9 older toddlers with 2 staff. When identifying their primary care groups, staff person #2 initially identified 4 children and staff person #3 identified 4 children. Staff person #2 then realized they missed one of the children in their group and identified 5 children in their group. During naptime in the Racoons classroom, there was a group of 4 older toddlers and 4 preschool children with 2 staff. When identifying their primary care groups, staff person #4 identified 4 children and staff person #5 knew they had 4 children in their group but did not know the names of the 4 children.

Correction Required: **Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. Operator will develop a supervision observation checklist. This checklist is to be approved by the Central Region Office of Child Development and Early Learning. The facility director and/or the assistant director will use the supervision observation checklist to conduct random daily monitorings in each classroom regarding supervision techniques. Completed checklists will be maintained on file by the facility director and be made available electronically to the Department Representative when requested. This will be done for a period of 3 months from the date this Plan of Correction is accepted by the Central Regional Office of Child Development and Early Learning.

When asked to identify their group, staff person #4 stated that she had 1 child and the remaining 3 children at the picnic tables were in staff person #6's group. Staff person #4 identified 2 other children in her group that were with staff person #6 at the basketball court. Staff person #6 and volunteer #7 were at the basketball court with 2 children. When asked to identify their group, staff person #6 stated that he was not responsible for any of the children and that all 6 children were assigned to staff person #4 and they were helping out. While at the basketball court talking to staff person #6, 1 of the children was observed running across the parking lot driveway to the basketball court from the picnic tables without a staff person accompanying them across the driveway. Observed staff person #8 and #9 were supervising a group of 16 preschool age children in the Dolphins classroom. When asked to identify their groups, staff person #8 initially named 5 children and staff person #9 identified 7 children and then named 8 of their children. When it was pointed out that there were 16 children in the room and only 12-13 were identified, staff person #9 then properly identified 9 children in their group. Staff person #8 then named 6 children and then realized there were 7 children in their group and were able to properly identify all 7 children in their group. Observed staff person #10 and #11 were supervising a mixed group of 2 preschool children and 8 older toddlers in the Pandas classroom. Staff person #10 identified 1 pk and 3 OT and staff person #11 identified 5 children in their group. When it was pointed out that there were 10 children in the classroom, staff person #11 properly identified their group of 6 children (5 OT and 1 PK).

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

The legal entity must arrange for all administration and facility staff to receive a minimum of two hours of in person training regarding supervision of children through the ELRC. The legal entity must receive DHS approval of the training content prior to scheduling the training.

Noncompliance Area: The file for staff person #3 did not include complete provisional hire documents prior to their first day working with children (see LIS code sheet). The file included documentation of the FBI fingerprinting receipt dated 08/31/2024 which was after the first day caring for children. The file included a request for the NSOR sent on 08/22/2024, a signed disclosure statement dated 08/20/2024, the results of the PA criminal history clearance dated 08/22/2024, Child Abuse Clearance dated 08/22/2024 and an FBI Clearance dated 09/03/2024.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person #3 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

While eating a snack at the pavilion inside the pool area, staff person #4 and #7 were observed with a group of 15 young and older school-age children exceeding the staff-child ratio of 1:6.

Correction Required: The staff-child ratios while children are swimming are Infant 1:1; Young or older toddler 1:2; Preschool 1:5; Young school-age 1:6; Older school-age1:8.

Description: Outdoor equip.- protective surfacing

Noncompliance Area: Observed on the school-age (back playground) a rope swing and rope ladder hung on trees which were not mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. Observed the swing and rope ladder were hung above a tree stump, creating a hazard.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.
