The outdoor premises were not clean, drained and free of litter and hazardous materials grass and other vegetation in a manner which does not encourage vermin. Vegetation growth was present in the fenced (chain link fence) area where the air conditioner is located. This is exactly located on the playground were children play, next to the handicap access.

The operator made an effort to falsify information. Upon request, Ms. McNeil provided sign in/out sheets for February 20 – 24, 2023 and parent contact information. When I contacted parents, they reported information different from what was reflected on the sign in/out sheets and therefore, it was determined Ms. McNeil provided false information to a representative of the DCDEE.

Surfacing did not extend six (6) feet beyond the external limits of the equipment or 3 feet for equipment used only by children less than 2 years of age. Fall zone surfacing did not measure six feet beyond the equipment.

All equipment and furnishings were not in good repair. Various pieces of equipment , including but not limited to Tonka trucks, bicycles, riding toys, and a play boat structure, were not in good repair. The Tonka trucks had peeling paint, exposing rust. A bicycle has a peeling seat. A toy boat structure had a loose piece that wasn't connected correctly.
