Noncompliance Area: On 10/07/20, During a renewal follow-up at the facility, Certification Representative observed an embedded outdoor playground structure. The slide on this structure was damaged with a large visible crack. Also, the bottom of the slide had collected a small puddle of rain water with dead bugs floating in it.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

The slide on the playground structure was sealed on the cracked area and made inaccessible to the children. In the future, we will inspect all playground equipment daily to ensure it is safe and free from hazards.

Noncompliance Area: During the renewal inspection on 10/05/20, Certification Representative observed 4 infants in the classroom with 1 staff member. One of the infant's positioning was behind a crib and out of the staff member's sight.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: The facility self-reported that on 9/8/20, at approximately 11:45 am, a child left the facility play space alone and onto the street walking a block away from the facility, crossing two streets, and arriving home, without staff being aware of his departure. The child was brought back to the facility by his parent at approx 11:55 am. Child was away from the facility approx. 10 minutes.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training.
