Noncompliance Area: The facility self-reported that on 9/8/20, at approximately 11:45 am, a child left the facility play space alone and onto the street walking a block away from the facility, crossing two streets, and arriving home, without staff being aware of his departure. The child was brought back to the facility by his parent at approx 11:55 am. Child was away from the facility approx. 10 minutes. Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children.
