Noncompliance Area: A renewal inspection occurred at the facility. A review of children's files was conducted. See the code sheet for the dates of enrollment. An operator is required to track children's growth and development using a form such as the child service report at least every 6 months. The following children have attended the facility for more than 6-months, and the operator has not yet completed a child service report on them: Child #1, #2 and #4.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3290.124(e).

Noncompliance Area: A renewal inspection occurred at the facility on 8/14/26. A review of files was conducted. At the time of the renewal inspection on 08/14/26 the state police clearance on file for Household Member #1 listed the clearance as "Under Review" and was not a completed clearance with a dissemination date. A new completed clearance was obtained on 8/16/26.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Household Member #1 will not be present in the home while children are in care.

Description: Parent home/work address, phone/Information re: disability

Noncompliance Area: The emergency contact form for the following children does not include the parent's work address or work telephone number: Child #1, Child #2 and Child #3. The emergency contact form for the following children does not include information on the child's special needs: Child #1, Child #2 and Child #3.

Correction Required: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. Emergency contact information must include information on the child's special needs, as specified by the child's parent, physician, physician's assistant of CRNP, which is needed in an emergency situation.

Description: Menus

Noncompliance Area: The operator has not discussed the menu with the parents.

Correction Required: The operator shall discuss the general menu plan with the parent so that the daily nutritional needs of the child can be met.

Description: System in compliance/Operator or designated staff responsibility

Noncompliance Area: During the initial inspection conducted on 9-5-25, the family home did not have an OSHA approved, interconnected smoke detection system on all three levels of the home. The smoke detectors on the first and second floor were interconnected, but the smoke detector in the basement was not interconnected

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met.
