In space #1, one (1) bottle of BIC Cover-it correction fluid with multiple safety warnings was observed in a cabinet secured with a safety latch but was not in locked storage with an approved locking device.

In space #2, leftover prescription medication (Triamcinolone Acetonide cream) for one child was not discarded or returned to the parent within seventy-two (72) hours. The medication authorization from the parent expired on May 12, 2023. In space #3, leftover medication (Aveeno Baby sunscreen) for one child was not discarded or returned to the parent within seventy-two (72) hours. The medication authorization from the parent expired on May 2, 2023.

Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. In space #2, visual sleep checks were not completed every fifteen minutes on four different occasions. On June 2, 2023, it was recorded that visual checks were conducted for one infant at 8:12am and not again until 8:30am. On June 5, 2023, it was recorded that visual checks were conducted for one infant at 12:20pm and not again until 12:45pm, and for another infant at 9:35am and not again until 10:10am. On June 9, 2023, it was recorded that visual checks were conducted for one infant at 3:35pm and not again until 4:43pm.

Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. One staff employed on May 25, 2023, did not complete orientation training within the first two weeks of employment until June 9, 2023. Only 4.5 hours of orientation were completed by June 8, 2023.
