Each staff had and is always provided tags for each child, including their full name, birthdate, age, and picture. Staff #3 had four tags but failed to match the tags with the children in care after a transition happened. Staff are now required to verbalize the names of the children in care to a manager or director who is present for each transition for at least the next month until they feel more comfortable completing transitions correctly.
