| Findings: Lack of Supervision/Ratio. At time of investigation,the facility was overcrowded and not within child-staff ratio. Children lacked proper supervision and were able to walk about unattended, bite each other and crawl into bookshelves. Also, safe sleep practices were not performed. In addition, there was a fan on the changing table while staff changed a child's diaper, the changing table was not cleaned after each use nor was proper handwashing practices used. Resolution: Provider will provide training for safe sleep, supervision, and proper diaper changing. | |||
