| A safe indoor and outdoor environment was not provided for the children. In Space 5, one child-sized vinyl chair and one child-sized vinyl ottoman had tears in the vinyl and worn edges. The fence on the playground designated to be used by preschool-aged children had gaps between the ground and the fence large enough for animals to crawl through and enter the playground space. The fence on the playground designated to be used by preschool-aged children had gaps between the fence support pole and the fencing where the fence had pulled away from the support pole. The metal playground equipment on both the playgrounds had chipping paint. Two concrete pillars on the building inside the playground designated to be used by infant and toddler aged children were deteriorating leaving sharp and jagged edges accessible to the children. | |||
