Incident reports for 12 children were stored with the incident logs and not with the child’s file.

The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. Medical Action Plans for 2 children were not filled out completely and did not contain all of the required information.

Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. One permission to administer form did not contain all the required information. One permission to administer form had standing orders that had expired on 12/6/2023.

Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Staff files for 2 teachers had health and medical information stored with personnel files. All medical forms for staff must be placed in a separate file upon hire and maintained away from other personnel records.

All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. One employee did not have a current Health Questionnaire on file for review. The employee completed the Health Questionnaire during my visit.

Emergency information did not name the child's health care professional. One child did not have the name of their health care professional indicated on their application. The parent completed the information on the application when they picked the child up today.

Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. A chronic medication permission form was written for twelve months instead of the allowed six months. The parent completed a new permission form today when she picked up her child.

One (1) medical action plan dated 3/12/2021 was kept in the Program Coordinator's office, but it was not attached to the student's enrollment application in the child record file and had not been updated annually.

Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. No arrival time was recorded for the fifty-four (54) students in attendance during today's visit.

Staff/child ratios applicable to a classroom, were not posted in each classroom. Classroom Staff to Child Ratio worksheets were not completed correctly. In Space 3, the gym stage area, the staff to child ratio worksheet reflected one (1) staff to twenty-five (25) students, but it should have reflected one (1) staff to twenty-four (24) students with a maximum space allowance of twenty-four (24) students. In Space 5, the cafeteria, the Classroom Staff to Child worksheet reflected the maximum space allowance was ninety-five (95), but it should have reflected ninety-four (94) as the maximum space allowance.

A drug or medicine was administered after its expiration date. Two (2) prescription medications were administered on May 3, 4, 5, 2022 after the two (2) medication permission to administer forms had expired on April 29, 2022.
