Initial Comments: The purpose of the inspection was to conduct a complaint investigation. A full inspection was not conducted at this time. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The ratios observed were: Infants: 2:6 1 year-old children: 1:6 / 2:4 2 year-old children: 1:5 / 2:6 3 year-old children: 1:3 / 1:10 / 2:21 There were 4 staff interviewed during this inspection. Other interviews: Director. Compliance Officer #1 attempted to contact the complainant via telephone on 12/12/2023 and via email on 12/15/2023, however was unable to contact the complainant. Documentation observed was rosters. Upon completion of the complaint investigation # 00066737, it was determined from observation, interviews, and documentation, the allegations were unsubstantiated. The following deficiencies were observed at the time of the complaint #0066737 investigation conducted on 12/19/2023 and are subject to changes pending programmatic review. Compliance Officer #1 is Patti Longman. Compliance Officer #2 is Stacy Marchelli.

Initial Comments: The following deficiencies were observed at the time of the Compliance inspection conducted on 10/5/2023. The Fingerprint Clearance cards for 5 of 5 staff members were verified to be valid on the DPS website during the inspection.
