Findings: Based on the Surveyor's observation upon arrival at F502, the Surveyor was not allowed immediate access to the activity area. The Surveyor knocked on the locked door 3 times. The Surveyor could hear staff and children in the room. Surveyor knocked again 2 minutes later with no response. The Surveyor waited 1 minute then banged on door jiggling the handle and calling out. At this time the Surveyor was allowed access to the activity area.

Findings: Based on facility documentation, staff interview and the Surveyor's observation, the Licensee lacked a posting, in all indoor activity areas, of the name of the Facility Director and the name of the individual designated to act on behalf of the facility director when the Facility Director. Room F502, Room F504, G602, the travel board and the main posting area all had a different person listed as the Director with the Designated agent as Designee. The Surveyor asked the Director how long the Director had been in the Director position and it was stated two years. The Director stated that the next person in charge should be Staff #1.

Findings: Based on the Surveyor's observation and staff interview, upon arrival at the facility, it was determined that the Licensee lacked a staff member to perform administrative duties. The Surveyor arrived at the school office and asked for the Director. The school office personnel informed the Surveyor that the individual was no longer in that position and contacted Staff #7 via telephone who stated she was the new Director and had just left for lunch. While the Director was offsite, there was no staff available to perform administrative duties.
