| Initial Comments: The following deficiencies were observed at the time of the Initial inspection conducted on 6/16/2025, and are subject to changes pending programmatic review. 3 of 3 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The following was discussed, but not limited to: 1) Submitting floor plans for the bathroom downstairs that is being added and the corrected floor plans for the upstairs bathroom regarding the correct number of sinks (6), 2) Corrected Statement of Services regarding the hours of operation for the aftercare program, 3) Add the director’s name to the licensing portal account, 4) Submitting current fire extinguisher inspection tags, and 5) Chemical substitutions for bleach. | |||
