Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. Two employees with dates of employment of 12/13/2022 and /3/14/2023 received four hours of training within the first two weeks and the training topic, Prevention of infectious disease control including immunizations, was omitted. One employee with a date of employment of 12/01/2022 did not complete new staff orientation within the first six weeks of employment. Orientation dated 2019 was used as new staff orientation training and documentation.

A valid Criminal Background Check qualification letter was not on file for two out of four therapists who serve children at the facility and supervise the children they serve while delivering therapy services outside the classroom.

Staff did not complete the health and safety training within one year of employment. One employee with a date of employment of 03/22/2022 did not complete the training topic, Prevention of sudden infant death syndrome and use of safe sleep practices, within the first year. A certificate for ITS-SIDS training dated 01/19/2019 was used as documentation that this training topic had been completed.

When three year old children and older were in care, the materials and equipment in the activity area was not in sufficient quantity to allow at least three children to use the area regardless of whether the children choose the same or different activities. In Room 229, the block area had two types of blocks for children to choose from but needs to have three types of blocks from which the children can choose to play.

The gate leading from the infant and toddler playground to the two and three year old playground had a gap between the gate and the support post measuring from 4 inches to 5 1/2 inches. The gate leading from the infant and toddler playground to the preschool and school-age playground had a gap between the gate and the support pole measuring 4 1/2 inches and between the gate and the ground measuring 5 1/2 inches.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. One aerosol can of disinfecting spray was stored in an unlocked underdash console space in the Ford van used to transport school-age children.

In Room 235, two permission to administer forms for over the counter topical ointments were missing the directions for how to apply the topical medication.

Each adult and child was not restrained by appropriate individual belt/device. Two car safety restraint seats used to transport school-age children in the facility van had expired on June 24, 2022. Three car safety restraint seats used to transport school-age children in the facility van had expired on August 27, 2022.

The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. Three staff members (GY, BP, and PJ) had a signed acknowledgement statement for the Prevention of Shaken Baby Syndrome and Abusive Head policy on file, but the acknowledgement statement did not include the required information.

I have used this provider for more than 6 months

This is the best childcare facility in the area. My two children just loved their teachers and all the staff are excellent, well trained, and caring. You can't go wrong with this center.
