In space 204 there were plastic packaged diapers and wipes stored unlocked underneath the diaper changing table because the magnetic lock no longer is working.

Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The fence surrounding most of the outdoor play area was measuring between 3 feet 7 inches and 3 feet 10 inches. There were also areas in which there were protrusions where the chain link fence at the top was not covered to prevent protrusions hazards.
