Program has operated and provided care to non-relative children since Christmas break, Dec. 30. No health inspection has been approved. no mention of location of compliance file in parent policies/handbook One personnel, owner's adult child caring for children has not completed background check process . Program does not have this form as they are not licensed at this time. Program does not have liability insurance or this exception form posted. Permit or license is not posted due to not being licensed at this time. NO physical environment checklist has been completed this time. Equipment inventory take was completed was not for the correct capacity. No personnel information sheets have been provided to licensing for all personnel. Personnel do not have verification of orientation completion. Director has not yet completed compliance review. Two non-relative children present today do not have any child records or documentation. Program has yet to provide information of director to licensing. No personnel was with the sleeping infant in infant, or sleeping two year old in two year old room upon arrival and walk through of the program. Personnel was not supervising sleeping infant and 2yr old in each room. 7 month old infant asleep in crib with blanket cover child. Light level in both rooms children were observed the child face or features could not be seen during walk through. |
