Plan: Owner/director reported she gave a permission slip to the parent of the diaper rash cream in the infant/toddler classroom and instructed parent to return the form with the cream. Parent dropped off diaper bag in classroom this morning and must have placed diaper rash cream in child's cubby. Owner/director said she was not even aware of the diaper rash cream in the pre-school room. Director will tell all staff to make sure that no new cream comes in and that they must have the form along with it.

Plan: Owner/director informed she didn't realize there were any diaper uses in that classroom and thought it was pull-ups only. She stated there is a changing table in the storage unit that she will get out; she also has a diaper changing pad available. Director will instruct staff that all diapers are to be changed on diaper changing table only.

Plan: Child fed in Jumperoo was staff's own child. Owner/director reported they normally set him in the Jumperoo while the other two highchairs are utilized and then they switch. The program's third highchair no longer had a strap so it was disposed of. Owner/director has ordered two new highchairs and they are on their way. They will be here by 6/22/21. Director will reiterate with staff that 1-year-olds need to be held or in a high chair for eating.

Plan: Owner/director stated she has spoken with parents and told them they must sign in and out each day; they just don't do it. They didn't have to in the past and don't feel like the should have to now. Owner/director said each classroom has a clipboard with child information for each child. It just doesn't have arrival and departure times. She has already modified her current form that is on the clipboard to include arrival and departure times, and if parents do not document times after signing child in, the staff will document times.
