SERIOUS-Incident occurred 2/24/2020 and was reported to director late in the day on 2/25/2020. Incident self-reported to Licensing on 2/26/2020. During transition time getting ready to leave the gym a 1yo child exited the gym and went into the Head Start safe room/dining room, where the child was met by kitchen staff and taken back to the gym. The child was unattended for 22 seconds.

Plan: The staff involved was written up, and was suspended for 5 days without pay. She was required to complete additional training on active supervision, and she reviewed the OKDHS orientation. An active supervision training is being held 6/18/19 for all staff. Director is in the process of surveying staff to assess their current understanding of supervision.

On 5/23/19 teacher assistant stepped across the hallway to open the door to speak to another teacher about a child, leaving three children unattended. One child followed the teacher out of the classroom, wandering into an open classroom and was left alone for approximately one minute before being found by a parent and another teacher.
