During transition time getting ready to leave the gym a 1yo child exited the gym and went into the Head Start safe room/dining room, where the child was met by kitchen staff and taken back to the gym. The child was unattended for 22 seconds. |

On 5/23/19 teacher assistant stepped across the hallway to open the door to speak to another teacher about a child, leaving three children unattended. One child followed the teacher out of the classroom, wandering into an open classroom and was left alone for approximately one minute before being found by a parent and another teacher. |
