| Findings: LS conducted a complaint inspection in which a parent stated that her 18 month old child fell while at the child care center on 08142023. She received a call to pick up the child and to take her to receive medical care. Parent was concerned that the story she was told about the fall is inconsistent with the bruising on the side of the child's face. The child's father requested the video from the center and was told he would have it in 24 hours and then he was told it would be another 24 hours. At the time of the inspection on 08/17/2023, the OCC nor the Licensing Specialist had been notified by the center that a child received medical attention for an injury. At the time of the inspection, the LS viewed the video and saw that the child tripped and fell and fell forward into the sensory table. Staff rendered immediate assistance and called the parent to pick up the child. The classroom teachers stated that they had a conversation with mom about the Childs shoes being too big and could have caused her to trip. | |||

| Findings: At the time of the inspection LS observed 26 children in Room 6 at the time of her arrival. Another staff member arrived about 10 minutes later and then another staff member arrived about 5 minutes after that and the children were taken to their rightful classrooms and the noncompliance was corrected. | |||
