Findings: There is one child's emergency card (A.A) who needs an authorized pick up person listed, another emergency card (J.C) needs to be signed and dated by the parents it is currently not signed or dated to know when it was completed. A third emergency card (E.I) needs to have an emergency contact listed in case the parents are not able to be contacted. These will be completed and sent to the licensing specialist as soon as possible.

Findings: The provider has hired a new aid but did not notify the office of child care. It was reviewed with the provider that a release of information, medical and prints must be sent to the licensing specialist before a new staff member starts. While on site the provider gave the employee a release of information and medical to complete. An updated staffing pattern was also provided to the licensing specialist.
