Noncompliance Area: On 2/20/20, it was observed that the hinge on the cabinet door of the changing table in the Preschool 2/3 classroom was broken.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: On 2/20/20, it was observed that staff person 4 (hire date 9/30/19) had a TB test on file dated 9/29/16 which is more than 12 months prior to their employment.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: On 2/20/20, it was observed that CPSL law was not followed correctly. Staff person 1 (hire date 1/13/20) did not have proof of submission of their FBI clearance until 1/14/2020. Staff person 2 (hire date 2/14/20) did not have proof of submission of their FBI clearance on file. Staff person 3 (hire date 9/25/19) did not have proof of submission of their FBI clearance on file until 9/30/19. Staff person 4 (hire date 9/30/19) did not have proof of submission of their FBI clearance on file until 10/2/19) All clearances must be submitted and provisional paperwork must be on file before a staff begins working with children.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person #2 -- may not work in a child care position at the facility until FBI Clearance is completed. .

Noncompliance Area: On 2/20/20, it was observed that staff person 2 was functioning as an Assistant Group Supervisor, but did not have verification of education on file.

Correction Required: A staff person or a substitute staff person shall meet one of the applicable staff qualifications for the position in which the person is performing. A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: On 2/20/20, it was observed that the floor in the Young Toddler classroom had holes in it.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: On 2/20/20, it was observed that there was peeling paint in the Older Toddler classroom on the low windowsill and around the door frame.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: On 2/19/19 it was observed that child 1 and 2 did not have a medication log with signed parent permission to administer the medication for the child that was on site.

Correction Required: A parent shall provide written consent for administration of medication or a special diet. An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered.

Noncompliance Area: On 2/19/19 it was observed that staff person 4 had not updated their physical in a timely manner. Staff person 4 had a physical on file that was dated 9/7/16 that expired on 9/7/18. A new physical was not obtained until 1/22/19.

Noncompliance Area: On 2/19/19 it was observed that staff 1, 2, and 3 did not have completed clearances or provisional hire at their start date with children. Staff person 5 did not have their clearances updated in a timely manner. Staff person 1 (start date 9/4/18) had a state police request and clearance date of 9/5/18, a child abuse clearance date of 9/16/19, and an FBI request date of 9/17/19. Staff person 2 (start date 3/14/18) had a state police request and clearance date of 3/15/18, a child abuse clearance date of 3/30/18, and an FBI clearance request date of 3/19/18. Staff person 3 (start date 6/1/18) had an FBI clearance dated 6/28/18 (no provisional paperwork was available). Staff person 5 had a state police clearance dated 8/28/13 that expired on 8/28/18. A new clearance was not received until 1/11/19. Staff person 5 had a child abuse clearance dated 9/10/13 that expired on 9/10/18. A new clearance was not received until 1/31/19. Staff person 5 had an FBI clearance on file dated 8/27/13 that expired on 8/27/18. A new clearance was not received until 1/17/19.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).

Noncompliance Area: On 2/19/19 a protruding screw was observed on the edge of the sink in the Older Toddler classroom. CORRECTED ON SITE.

Noncompliance Area: On March 15, 2018 it was observed that the door to the kitchen in the 2/3 year old classroom was shut, but not secured. There is a stove located in the kitchen. CORRECTED ON SITE.

Correction Required: Hot water pipes and other sources of heat exceeding 110 F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact.

Noncompliance Area: Child 1 had an epi pen on site that was expired-CORRECTED ON SITE

Correction Required: Medication shall be stored in accordance with the manufacturer`s or health professional`s instructions on the original label.

Noncompliance Area: Child 2 did not have a medication log on file and had an epi pen on site.

Correction Required: An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered.
