Findings: The complaint states that the caller stated that employees are completing background checks very late after already working at the center for several months around the children. During the inspection, LS observed a volunteer at the center for whom the OCC does not have proof of compliance with the laws and regulations pertaining to criminal background checks, and signed and notarized permission to examine records of abuse and neglect of children and adults for information about the individual. This individual was also observed during the mandatory inspection on 05/18/2026, and the director was informed that the individual should not be in the center until we have received clearances.

Findings: LS conducted a complaint investigation on today's date. The complaint states that the caller has observed the center having 35 children and the bathroom ratio is off. Upon arrival at the center, LS observed 18 children at the center. LS observed a total of 9 children inside the center. LS asked the director if there were currently any other children present at the center, and the director informed LS that there were no other children present. LS then looked outside in the outdoor activity area, and observed 9 more children present. The center capacity approved on the license issued by the OCC is 15. An operator shall limit the total number of children in care at one time to the capacity approved by the office.

Findings: The complaint states that the center is over capacity and that the bathroom ratio is off, and that staff are taking children to a store across the street to use public restrooms. Upon arrival at the center, LS observed 18 children at the center. The center capacity approved on the license issued by the OCC is 15 because the center currently only has one sink and one toilet. For every 15 children who are 2 years old or older, an operator shall provide one toilet and one sink.

Findings: During the inspection, LS observed two individuals that work for the transportation company that transports the children to and from the center, come inside the back of the center to use the children's restroom. The center currently does not have a staff restroom because of the hours of operation, and upon licensure, the center was informed that adults and staff would need to use the bathroom at another location. A center licensed for the first time shall provide at least one toilet facility restricted to use by adults that is equipped with a toilet, sink, and toilet supplies, except that this requirement does not apply to small centers or programs that operate 2 and 1/2 hours or less per session per day.
