| Findings: LS interviewed director, who advised that the child referenced in the incident got out of the playground gate on the left side of the center facing the street. LS observed that fence was secured by a latch, and difficult to open. The director believes that the child may have climbed over the fence. LS also interviewed the aide that retrieved that child, who stated that she was walking the playground and noticed that the child was outside of the fence standing near the road. The aide advised that staff are all assigned stations on the playground, but the teacher that is normally assigned to stand by the fence, was inside doing lunch breaks. | |||
