All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. Located in the Ford Transit Van, in the back of the passenger seat pocket was a bottle of Whipped Vanilla Soft Cashmere Shimmer on Skin perfume.

Health and safety trainings in the following topic areas have not been completed within the past 5 years, prevention and control of infectious diseases; building and physical premises safety; prevention of shaken baby, abusive head trauma, and child maltreatment; emergency preparedness and response planning; handling and storage of hazardous materials; and precautions in transporting children.
