Noncompliance Area: An approximately 36"x18" standing, two-door cupboard was observed to have a protruding screw (exposed rounded end) on the inside of the left door, in an area accessible to children in care. The protruding screw could potentially cause laceration injuries.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: In the pre-school room, two approximately 5"x12" plastic storage containers were observed to have broken, sharp edges. One similar bin was observed in the infant room to have a cracked, jagged corner. Each of the noted bins were in areas accessible to children in care. Damaged areas of bins could potentially cause laceration or puncture injury.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The file for Staff #1 contained a PA State Police Criminal Record Check that listed the purpose of the clearance request as Volunteer.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1-- may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1--- may not work in a child care position at the facility.

Noncompliance Area: In the first restroom in the back hallway, which is used by children in care, a rusted, exposed bolt was observed protruding from the base of the toilet.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: A hands-free trash can was observed in the infant area that is used for the disposal of diapers. The hands-free mechanism on the receptacle was not functioning.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Noncompliance Area: Bleach wipes and a spray bottle containing cleaning solution were observed in a cabinet in the preschool room. The cabinet was equipped with a lock; however, the locking mechanism was not functioning properly making the contents accessible to children in care.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: Two preschool-aged children were observed sleeping in spaces between a wall and two storage shelves that were positioned on either side of the children, perpendicular to the wall. Both children's cots lacked two feet of space on three sides.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Noncompliance Area: During nap time, Staff #1 identified seven pre-school-aged children as the group for whom she was responsible for supervising. Staff #1 was seated rubbing the back of a child who was lying on rest equipment. She identified two other pre-school-aged children in her group who were sleeping on rest equipment in an area that could not be visually monitored by Staff #1 as there was shelving between Staff #1 and the two previously-identified preschoolers.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: Three cribs in the infant room were observed to have loosely fitting mattress sheets that could lead to the gathering of material around a sleeping infant.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: A group of children was observed in the outside play space who were being supervised by two staff members. One of the two staff members was observed to escort two chidlren from the outside play space to the inside area of the facility. one of the chidlren was observed to have had a bathroom accident. The one staff member who was outside with the remainder of the children was observed to be supervising eight children between the ages of 1 and 3. The staff-child ratio for children between the ages of one and two is one staff member for every five chidlren. The second staff members, whom had escorted the two children to the inside of the facility, returned to outside area approximately 4-5 minutes after leaving the area.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level).

Noncompliance Area: An unlabeled spray bottle that, according to the present staff member, contained a bleach/water solution was observed in a cupboard in the infant room that was inaccessible to children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: The glass, double-doored entry way to the facility lacked visual identifiers on the lower sections of each door.

Correction Required: A visual strip or other visual identification shall be placed on glass located in a traffic area, a child care space or a play space.

Noncompliance Area: In the toddler room a plastic storage container used for storing a train set was cracked creating a sharp edge.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Child file #1, a preschool aged child, contained a most recent health report completed more than 12 months ago.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Noncompliance Area: In the toddler room the sink in the diapering area was not functioning. Staff disclosed that after diapering, children's hands are sometimes washed with a wipe which is not an acceptable form of handwashing.

Correction Required: A source of running water for handwashing shall be present in infant and toddler diapering areas. If the running water does not flow directly into a drain that is connected to a sewage system, a receptacle shall be provided to contain the water used for washing. The receptacle shall be emptied into an approved sewage system at least once a day.A staff person shall ensure that a child`s hands are washed before meals and snacks, after toileting and after being diapered.
