Noncompliance Area: At the time of inspection, 19 children between the ages of three and five were observed. During nap time, two of the 19 children were observed lying on mats in an area that did not have a minimum of two feet on three sides.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Noncompliance Area: In the child care space, in a locked cabinet above the sink, an unlabeled spray bottle was observed. The contents of said bottle had suds at the surface indicating that the spray bottle contained an unidentified cleaning solution.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: An approximately 42" wooden coat rack was observed in an area accessible to children in care. One of the wooden peg hangers on the rack was observed to be splintering, which presented the potential for laceration and/or puncture wounds.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: In an area accessible to children in care, several dime-sized areas of peeling paint were observed approximately three feet from floor level in the rear right corner of the child care space relative to the room entrance.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: In the outdoor play space, the chain link fencing that encloses the play space was observed to have numerous flexible aluminum wire fasteners that have sharp ends that are accessible to children in care. Also on said fencing, a bolt was observed on a fence post to the right of a shed that points inward towards the play space and is accessible to child in care.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: A tape dispenser with a sharp, serrated metal edge was observed in an area accessible to children in care.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: Several aluminum clips with sharp ends in areas accessible to children in care were observed on the chain link fence enclosing the outdoor play space. A wooden shed in the outdoor play space was observed to have a splintering, jagged piece of wood framing below the door of the shed in an area accessiible to children in care.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility`s outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: On a shelf in the block area plastic animals were being stored in a cracked plastic bin with a sharp edge.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Staff file #1 contained an initial health asessment and TB test dated after her employment start date.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.
