Noncompliance Area: The file for Child #1, who is five-years-old, contained an immunization record; however, said record listed only two of the four PCV 13 recommended by the ACIP. Per ACIP recommendations, the fourth PCV 13 vaccine should be administered by the age of 15 months.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP.

Noncompliance Area: The files for Staff #1, #2, and #3 contained documentation of the completion of annual emergency plan training; however, the dates that said training was completed was not listed.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: Although available on file, the facility's emergency plan was not posted in a conspicuous location.

Correction Required: The operator shall post the emergency plan in the facility at a conspicuous location.

Noncompliance Area: An unlabeled spray bottle containing a liquid that was later identified as soapy water was located in a secured cabinet under the sink.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: Exposed, protruding bolts were observed on both sides of a rectangular water play table in an area accessible to children in care. Contact with the bolts could cause laceration and/or puncture injuries.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: In an area accessible to children in care, two approximately 8" x 2" wooden toys were observed to have broken ends . The broken edges of each of the toys created the potential for a child in care to get a splinter or laceration.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: A roughly 2/3 full 16 ounce container of Acetone/nail polish remover was observed in a cupboard on a shelf above the sink in the child care space. The container was being stored on a shelf above paper plates and an apple slicer used to cut/mash apples for apple sauce that is consumed by children in care. There is potential for the Acetone to leak down onto the plates and/or apple slices, which could contaminate noted items and, in turn, be consumed by children. The container of Acetone is labeled Keep Out of Reach of Children.

Correction Required: Cleaning materials and other toxic materials shall be used in a way that does not contaminate play surfaces, food, food preparation areas and does not constitute a hazard to the children.

Noncompliance Area: During naptime, 17 preschool-aged children were observed sleeping/lying on cots throughout the classroom. Seven of these children's cots were positioned with less than two feet of spacing on three sides.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Noncompliance Area: A cable/wire connected to the wall-mounted television was observed hanging down into an area that is accessible to children in care. The other end of the cable/wire was secured at the other end, which created the potential for the cable/wire to be looped, which presents a strangulation hazard.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: A surge protector near the teacher's desk, in an area accessible to children under the age of six, was observed to have two unused, uncovered electrical outlets.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: The hot water temperature in the bathroom used by children in care was measured at 116 degrees Fahrenheit.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.

Noncompliance Area: The files for Child #1, #2, #3, #4, and #5 contained agreement forms that did not reflect the admission dates of the children.

Correction Required: An agreement shall specify the date of the child`s admission.

Noncompliance Area: Files for Child #1 and #4 included emergency contact forms that did not reflect the phone numbers or addresses for persons to whom the child can be released. The files for Child #2, #3, and #5 included emergency contact forms that did not reflect the addresses of persons to whom the child can be released.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: Files for Child #1, #2, #4, and #5 contained emergency contact forms that did not have a parental signature giving consent for emergency medical care; nor did said emergency contact forms have a parent's signature giving consent for minor first aid.

Correction Required: A child`s record shall contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission.A child`s record shall contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

Noncompliance Area: Child file #1 did not contain an initial health report and has exceeded 60 days since the first day of attendance.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.
