I have used this provider for more than 6 months

When I was initially looking for a daycare, I explained to the owner that my child is very premature & I was nervous about the whole daycare situation. She reassured me that her staff does individualized care planning per child. These past couple times I went to pick her up, my child was always missing her belongings. This daycare staff is very unresponsible & careless! There is no reason as to why another child’s binky was in my daughters mouth! I called the daycare immediately & asked to speak to the owner. She didn’t want to get on the phone, instead she has someone relaying messages back & forth. She said she was going to call me back & still never did! No apology, no nothing. Not to mention, it’s very unsettling when you’re calling trying to check on your child & no one at the daycare answers the phone! I would not recommend this daycare, the owner is very unprofessional! Too many things happen at this daycare & go unnoticed!

It was determined from these sources that the following events occurred: Facility Person #1 transported the child to the facility at which time the child fell asleep. After dropping off the other children on the bus at the front entrance to the facility, Facility Person #1 parked the bus behind the building and left the vehicle at 3:12 PM. There were no other staff on or near the bus at this time. At approximately 3:25 PM, Facility Person #2, while parking another vehicle behind the facility, saw the child, who was now awake, on the bus. Facility Person #2 determined the bus was locked, obtained the key to the bus from inside the facility, and retrieved the child at 3:28 PM.

Correction Required: 1.Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. This portion of the plan shall have an immediate correction date. 2. All facility persons, including the owner, director, staff and drivers, shall participate in an onsite, in-person training session with the ELRC #1 on the topic of supervision, with a focus on supervision prior to departure, en route, and upon arrival when transporting children. This training can count towards the required annual trainings described in 3270.31(e) upon completion and submission of acceptable verification. This provider shall provide a date for when this training will be completed. 3. Subsequent to training and in conjunction with ELRC #1, the owner and the director will create a step-by-step transportation procedures/ policy to include duties of staff/ facility persons, including drivers, prior to departure, en route, and upon arrival both to/from the facility. This policy shall include a checklist of duties to be completed each time the facility transports; copies of this checklist shall be posted in each of the vehicles. The facility will consult with the ELRC #1 for assistance and suggestions. The facility shall submit this policy to the Regional Office for review prior to use. The operator shall provide a date for completion of this step, which shall occur after the completion of the training outlined in Step #2 of the required correction. 4. Upon DHS review of the policy, facility persons, including the owner, director, staff and drivers will participate a minimum of 2 onsite, in-person technical assistance sessions with the ELRC #1 to further refine and review the transportation supervision and implementation of the policy. The operator shall provide a date for completion of this step, which shall occur after the completion and review of the policy and checklist development outlined Step #3 of the required correction. 5. For a period of 3 months, the owner or director will assess the transportation procedures and policies at least 3 times weekly by observing staff/ facility persons/ drivers prior to departures from the facility and upon arrivals to the facility. A minimum of one check per week shall be conducted by the owner of the facility. The owner/director will use the checklist established in Step #4 to assess whether or not staff completed all steps. They shall provide notes on the checklist, and sign and date the checklist each time transportation is assessed. All completed transportation checklists will be kept on file at the facility and will be provided (either physically or electronically) at the request of the Certification Representative. The correction date for this portion of the plan shall be a minimum of 3 months from the date that the second required technical assistance session has been completed.
