I have used this provider for more than 6 months

I’ve used this facility for my child over a year now. Where to start, if I could give no stars I would. Within the year my child has caught Hand, Foot, Mouth Disease and passed it to me. This facility always smells like poop upon entering. My child has been bitten by another child and I was not notified til hours later. My child was pushed to floor by another child hitting head leaving a knot. The staff are nice until an incident happens. The front desk staff are unprofessional and need training on business ethics. The assistant supervisor gives discounts to who she wants to. This place has changed drastically since the last Director and I foresee this facility closing due to uncleanliness, ethic violations and bias judgments towards clients.

I have used this provider for more than 6 months

I have never had such an unpleasant experience involving my child. the people running this beautiful facility do not care for the children! at over 200 a week there are no fresh veggies, No fresh fruit. the front desk personnel are extremely unprofessional! upon entering it always smells like feces. they allow the children to fight and in one incident the child hurting mines was that of a worker and nothing was done because of that fact. they do not clean ad wipe the surfaces down. a day before tuition is do the assistant director becomes extremely nasty and aggressive! makes comments in front of children and other parents etc. do yourself a favor and spend your hard earned money elsewhere. the behavior of these woman was disgusting.

Findings: The complaint alleges a staff person is picking up and moving infants with one hand while using a cane with the other hand to support herself. It was reported the staff person has limited mobility and sits in a chair above the children then leans over to tend to the infants. There are concerns if the staff person falls over onto the infants or how the staff person can safely the infants in an emergency. At the time of the inspection, the staff member was alone in Infant A with 3 infants. She was observed using the shelf to support herself and the cane for support when moving around the room. The staff person uses a chair in the room to tend to the infants who are on the floor. In order to feed one infant a bottle, the staff person moved the chair near the infant then was able to use both arms to lift the infant to her lap. One infant was crying for a bottle but the staff person was switching cribs for an infant that can roll over and she needed the crib for support during the task. It was reported that she calls for assistance with diapering and moving infants in and out of cribs; however, this did not appear to be happening each time.

Findings: The complaint alleges infants are sleeping on the floor and not being moved to the cribs after falling asleep because the staff person is unable to pick the infants up and move them to the cribs. It was reported that an infant was asleep on the floor on its belly for 30 minutes and another infant fell asleep upright in a Bumbo seat. Three times during the first 3 hours of the inspection, infants were observed sleeping on the floor (on Boppy pillows and in chairs). The first time it was stated the child just fell asleep and assistance was requested by the staff person to place the infant in her crib. Around 12:00 PM, two infants were observed asleep in unapproved sleep sources. One child was in an almost sitting position but asleep with the infant's neck bent back. This was brought to the director's attention and as it was the staff person called for assistance with moving the infants to the cribs. The staff person is also unable to lift the infants from their crib to take them out.

Findings: The center self reported an incident in which a staff member was observed shaking a two year old child during a temper tantrum. The staff allegedly held the child on the ground while the child was kicking and screaming then lifted the child in the air. She proceeded to yell at him and shake him before again pinning him back to the ground. The staff's actions were reported to be excessive by witnesses. It was reported that there were no observable injuries and the child was crying a lot. The staff person was placed on administrative leave after the incident and terminated as of March 21st. The accused staff person was interviewed at her home. She admitted to grabbing the child, picking him up and yelling in his face to stop. This was in response to the child having an upset morning and throwing temper tantrums. She reported the child was kicking and almost knocking shelves over as well as trying to bang his head on the floor and wall. She said she picked him up by his arm/shoulders and sat him hard on the floor. She also admitted to aggressively yanking the child and shaking him to try and keep him from kicking her. (CONT)

Findings: The complaint alleges the staff are mopping floors while children in care and children are walking on the floor while still wet. A staff person stated that staff are required to do daily cleaning and the cleaning checklists are on a clipboard in every classroom. They are required to wipe down tables and chairs then spray the toys and cots. She stated she does mop every evening around 6:10 PM when they close at 6:30 PM. She said some children may still be in care and are doing activities on the carpet. She was advised by the licensing specialist that she must wait until all children are gone to do that type of cleaning (mopping the tile floor) so that they are not exposed to the cleaning product or a slipping hazard is present. No mop buckets, cleaning supplies or trash were observed in the hallways. A cleaning company does come in and clean two or three times each week. One staff person stated that she had to purchase her own disinfecting spray because she did not put it on the supplies needed list. The center should still supply all cleaning products.

Two licensing specialists were on site at that time on 1/29/2024 and observed all rooms. The specialists arrived around 11:38 AM and one specialist immediately started to look in all of the classrooms. In both of the two year old classrooms, there were 6 children present with one staff person. Preschool I had a special needs child with a one-on-one caregiver. At the time of today's unannounced inspection, there were 7 infants observed in Infant B with 2 staff persons. The staff stated that they alerted administration but nobody had come to move the extra infant yet. The classroom was observed after checking all classrooms and one infant had been moved to Infant C so the group size and ratios had been corrected.

Findings: The complaint alleges an 8 year old boy was physically assaulted by 3 other children in the school age room on the afternoon of January 24th. The complainant stated she was notified via a staff person that evening and not by the center at pick up. It is claimed that a staff person's school age daughter hit her son in the head multiple times while he was sitting at a table. Then 2 boys were called over by the girl and they began to punch and kick the child until he fell to the ground. One of the boys is the director's son according to the complainant. Her son was able to get up and run to the bathroom. The complainant feels nothing is being done and the center is not taking the incident seriously because the aggressors are staff children. The director telephoned the specialist to let her know that a complaint may be coming in about a child allegedly being jumped in the school age classroom. Adequate supervision was not being maintained if up to 6 out of 14 children were involved in an altercation and the staff person did not observe anything until the child was crying in the bathroom. The school age classroom is one large square room with good visibility.

Licensing Specialist made contact with several staff at the center. Three of the infant staff who were interviewed separately report that on the morning of Friday May 13 there were 7-8 infants in the Infant B room with 2 staff. They report that his happens often in the mornings, especially Mondays, during drop off time when there are not enough staff present to accept the children in the classrooms and complete bus runs. The administrator on duty today during the inspection reported that this does not happen when she opens, as she ensures that the classrooms are in ratio. If the classrooms are going to be over ratio, she will put the children in a buggy and push them around or keep them at the desk with her for the few minutes it takes for the next staff to arrive.

Findings: The OCC received a compliant concerning an infant being left alone in a classroom while sleeping. Three staff who were present at the time of the incident were interviewed. The three staff were at the front desk clocking out. All three staff indicated that the staff person in charge of the care of the infant walked out of the classroom and came to the front desk of the center, asking where she was needed next. The administrator asked the staff person if her classroom was empty. The staff person indicated that she had an infant sleeping in her classroom. The administrator informed the staff person that she was not permitted to leave her classroom if there were still children in the classroom, regardless if they were asleep or not. The staff person went back to the classroom. The infant was alone in the classroom for approximately 2-4 minutes. The staff person is new and indicated that she had forgotten that she had a child in her classroom and did not know she could not leave the sleeping baby alone. The staff person was off today and not at the center to be interviewed.

Findings: The OCC received a phone call from a concerned parent who witnessed on the classroom cameras a teacher pull a chair out from under her child causing the child to fall in the floor. The parent reported that the other teacher in the room yanked her child's arm and forcefully put him on his bed. The parent sent the video to the Licensing Specialist to view. The Licensing Specialist saw on the video one teacher walk by the child sitting in a chair and pull the chair out from under the child, causing the child to fall in the floor, and then just walk away. The video continued and showed a second teacher grabbing the child's arm and moving him from one spot to another and then forcing him down by his arm. Contact was made with the director at the center 48 hours later. The written statements from the two staff were given to the Licensing Specialist. One of the staff was present at the center and one has been put on leave pending the investigation. The video was viewed more clearly by the Licensing Specialist again and the one staff present was interviewed. The behavior and discipline displayed by both staff in the video was clearly inappropriate. See details in the remarks section.

Findings: The Office of Child Care received a call from the center director notifying the Licensing Specialist of an incident that occurred at the center. A three year old child was left in the bathroom of City Place by himself for approximately 15 minutes. The management staff person and two other staff persons did not follow the procedure for moving the children from one room to another, which consists of taking a count of the children in the classroom where they pick the children up from, doing a sweep of the room, and taking a count of the children in the classroom where they are dropping off the children. Management signs off on the movement log, which is on a clip board that is taken back and forth between the classrooms when they move. The director sent written statements from each of the staff involved to the Licensing Specialist. The Licensing Specialist made contact with the director and two staff who were involved at the center today. All were interviewed separately. All indicated that the procedure was not followed and the child was left alone for a significant amount of time. Corporate and the director have both disciplined all staff involved for the incident.

Please ensure that cleaning is done regularly. The carpet in Toddler C and the 2s classrooms needs to be replaced - it is very dirty, smells bad, and seems that cleaning attempts are not resolving the issue.
