| Findings: A complaint was received that a 5 year old child was left unattended on the playground. It was reported that teacher W.F.C. and aide, K.C., brought the class outside at 10:15am and went back in at 10:40am. At 11:05am the director was contacted by T.S., a teacher from a different classroom , who informed the director that she noticed a child outside on the playground. T.S. retrieved the child from the playground. The director came to T.S.'s classroom and took the child back to her assigned classroom. Neither staff were aware that the child was not in their classroom. At this inspection aide, K.C., director, A.S., assistant director, J.H. and teacher T.S. were interviewed. The video footage from that day was also reviewed. The video footage showed the following time line: 9:50- class goes outside (a total of 11 children) 10:20 the child goes in the playhouse; 10:23 children begin to line up around K.C.; 10:24 10 children are seen going into the classroom from the outside door; 10:42 teacher T.S., calls the child, who was left outside, to come in her classroom.
