Findings: A complaint was received that a 5 year old child was left unattended on the playground. It was reported that teacher W.F.C. and aide, K.C., brought the class outside at 10:15am and went back in at 10:40am. At 11:05am the director was contacted by T.S., a teacher from a different classroom , who informed the director that she noticed a child outside on the playground. T.S. retrieved the child from the playground. The director came to T.S.'s classroom and took the child back to her assigned classroom. Neither staff were aware that the child was not in their classroom. At this inspection aide, K.C., director, A.S., assistant director, J.H. and teacher T.S. were interviewed. The video footage from that day was also reviewed. The video footage showed the following time line: 9:50- class goes outside (a total of 11 children) 10:20 the child goes in the playhouse; 10:23 children begin to line up around K.C.; 10:24 10 children are seen going into the classroom from the outside door; 10:42 teacher T.S., calls the child, who was left outside, to come in her classroom. Findings: The staffing patterns that were posted in the rooms were not accurate. The director will need to update the staffing patterns, post them in the classroom and send copies to the licensing specialist.

Findings: The licensing specialist observed a loosely fitted sheet on one of the cribs, however, during the inspection the teacher switched the sheet for the tightly fitted sheet. It is advised the loosely fitted sheets be taken out of the center so they are not used.
