Date Type Regulations Status
2022-03-16 Complaint 13A.16.08.01A(2)(a) Corrected
Findings: Regulation discussed with Center due to incident that occurred in which one staff member was with a group of children, but left them alone in the classroom to move their belongings into the hallway for preparation of dismissal. The staff member reported that she propped the door open. It is estimated that she was out of the physical classroom for approximately one minute. However, during that period of time, a child bit another child. Center has already changed their procedure to prevent this from happening in the future. They are now keeping backpacks and belongings in the classroom until the child's parent is picking up the child. The backpack and belongings will then go out of the classroom with the child.
2022-03-16 Complaint 13A.16.08.03C(2) Corrected
Findings: During the inspection, the Center was over the maximum group size In both the preschool room 2 and the preschool room 3. Pre-school room 2 had 22 children present and 22 children enrolled. Pre-school room 3 had 21 children present and 21 children enrolled. Licensing Specialist instructed Director to move two children from PS room 2 and one child from PS room 3 to PS room 1. Therefore, the current non-compliance was corrected on site. However, the number of children enrolled in pre-school rooms 2 and 3 remain over the maximum group size if they are all present on the same day. Please write a letter of correction stating how the Center will become in compliance with group size on an on-going basis.
2022-03-16 Complaint 13A.16.09.04F Corrected
Findings: During a complaint inspection, 4 cribs in the Infant B room were observed to have very loosely fitting sheets. This is a safety hazard and is unacceptable. When brought to the Aide's attention, she reported that they had other sheets in the closet. She replaced the sheets brought by the parents that were not snugly fitting with Center sheets that are snugly fitting. Therefore, the non-compliance was corrected on-site.
2021-07-20 Full 13A.16.03.03B(1) Corrected
Findings: While assessing the number and the ages of children in the Preschool 2 classroom, it was determined that one of the children had not been signed in as being in attendance. Staff said that he needs to be activated in the computer system to show up on their tablets. Staff admitted that he had been in their classroom for "weeks". Staff should be able to determine the whereabouts of each child at all times. If a child is not recorded as being in attendance, staff cannot account for him and his whereabouts. Please write a letter stating how the Center will become in compliance with this regulation in the future.
2021-07-20 Full 13A.16.03.03B(4) Corrected
Findings: During the in-person inspection, there was no evidence of a fire drill occurring in 08/2020, 10/2020, 11/2020 or 06/2021. Make sure you are conducting a disaster drill at least twice a year and a fire drill once a month and recording each. One does not count for the other. They are different types of drills and should be conducted as such.

Findings: During the in-person inspection, it was noted that the posted staffing patterns were inaccurate. Some of them did not accurately reflect which staff were in the room and some reflected staff in the room at the same time they were reflected as being in another room. Please make sure staffing patterns are posted in each room, that they are accurate, do not "overbook"/overlap staff room to room and that the staff are qualified for that age group. Please submit accurate staffing patterns to OCC that are appropriate for each room.

2020-07-08 Mandatory Review 13A.16.06.09B(1) Open
Findings: Center has several preschool teachers and was given a list of what trainings are needed by what teachers. Center is getting a non-compliance due to one of the nine teachers not having completed their training as required. One of the nine teachers has been employed less than one year and the other eight have until December 31,2020 to compete their training due to the COVID-19. See remarks sections for initials of teacher needing to complete her required training hours.

Do NOT take your infant to this center! Not only did they place my 4 month on old his belly to nap they didn't feed him for 5 hours. When we complained to the director she had the nerve to tell us that my son peed on the teacher and it "rattled her"! So she decided not to fee him?! WTF?! My son has not been back. I do not recommend this facility. We weren't even given an apology!
