Findings: The complainant observed fourteen 2-year-olds with 2 staff members in the Preppers A classroom and thirteen children with 1 staff member in the Preschool 2 classroom. On the day of the inspection, the director shared that 2 and 3-year-olds were combined due to the absence of 4 staff members. The Licensing Specialist noted that there were four 2-year-olds and fourteen 3-year-olds with 2 staff members in the Preschool 2 classroom. When staff member, VP arrived at the center the noncompliance was corrected as there were 3 staff members with the mixed age group of eighteen 2 and 3-year-olds. All the other classrooms were observed to have the correct staff/child ratios.

Findings: The Licensing Specialist interviewed the staff in regards to their cleaning procedures, specifically the sanitation and disinfection of toys. The staff shared that they follow the center's cleaning guidelines such as utilizing a toy bath. The toy bath is to be used during naptime and is prepared with a bleach and water solution. The toys are then placed on dry paper towels to air dry. The director stated that the center uses a bleach solution with the proper bleach to water ratios in their spray bottles. The complainant reported that her daughter's pants and shirt had been bleached which caused dry patches to appear on her skin. The director shared that the complainant's daughter had played with plastic blocks the day her clothing got bleached. After the Licensing Specialist and director inspected the blocks closely, it was discovered that they still had a small amount of bleach solution inside of them due to having a minuscule hole on one side of each block. The director shared that the plastic blocks that retained the bleach and water solution have been removed from the Preppers A classroom and will not be used again.

Findings: VL and HN were working in a classroom during the time of the inspection. The OCC was not notified that these two staff had been hired. Therefore, VL and HN have not been fully cleared by the OCC to work with the children. VL and HN left the premises during the inspection per the Licensing Specialist's request. The OCC was also not notified of staff member, VP. VP is no longer an employee of the center as of today. Please have HN complete and submit a notarized release of information to the OCC. The Licensing Specialist received VL's release of information during the inspection. VL and HN shall not return to the center until their criminal background checks and abuse and neglect clearances have been received and cleared by the OCC.

Findings: Upon inspection of the Preschool 1 classroom the Licensing Specialist noted that there were five 2-year-olds in a mixed aged classroom with thirteen 3-year-olds. One 2-year-old was taken to the Preschool 2A classroom during the inspection. When the 2-year-old was moved, the staff-to-child ratio was corrected.

Findings: Upon entering the Preschool 2 classroom the Licensing Specialist noted that there was only 1 staff person with 14 preschoolers. When the Licensing Specialist brought the insufficient staff/child ratio to the acting director's attention she immediately had 4 preschoolers from the Preschool 2 classroom move to the Preschool 3 classroom making it so that 2 staff had a total of 19 preschoolers.

Findings: Upon entering the Infant A classroom the Licensing Specialist observed that a 3 month old's arms were restricted while sleeping in a sleep sack. The Licensing Specialist explained to the acting director and the staff in the Infant A classroom that sleep sacks are only permitted if the infant's arms are not restricted and able to flow freely. During the inspection a staff person let the infant continue to use the sleep sack, but immediately took the infant's arms out of the sleep sack so they were not restricted.

Findings: Regulation discussed with Center due to incident that occurred in which one staff member was with a group of children, but left them alone in the classroom to move their belongings into the hallway for preparation of dismissal. The staff member reported that she propped the door open. It is estimated that she was out of the physical classroom for approximately one minute. However, during that period of time, a child bit another child. Center has already changed their procedure to prevent this from happening in the future. They are now keeping backpacks and belongings in the classroom until the child's parent is picking up the child. The backpack and belongings will then go out of the classroom with the child.

Do NOT take your infant to this center! Not only did they place my 4 month on old his belly to nap they didn't feed him for 5 hours. When we complained to the director she had the nerve to tell us that my son peed on the teacher and it "rattled her"! So she decided not to fee him?! WTF?! My son has not been back. I do not recommend this facility. We weren't even given an apology!
