The bench located on the outdoor play area is peeling yellow paint.

There was no documentation, with the required information, on file that parents reviewed the discipline policy for a child enrolled 7/11/22.

Operator did not provide a written statement to parents regarding the smoking and tobacco restriction. There was no documentation that the smoking restriction policy was reviewed with parents for one (1) child's file.

Prior to enrollment of children in a Family Child Care Home, and before animals come into the Family Child Care Home, the operator did not obtain each parent's signature on a form acknowledging the type of animal located on the premises and where the animal will be kept during child care operating hours and /or the signed form was not maintained in the child's file. There was no animal acknowledgment form on file for any enrolled children.

The Family Child Care Home operator did not discuss the operational policies with parents on or before the child's first day of attendance. There was no documentation that operational policies were discussed/reviewed with parents prior to enrollment for six (6) children's files.

Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser were not in their original container or in another labeled container, used according to manufacturer's instructions. A container of bleach was stored on a shelf above the cubbies.
