The window shield on a toy motorcycle was cracked exposing sharp edges, an unraveled water hose was laying across the yard leading to the fenced in play area, and standing water was observed in pots and toys on the playground.

A can of air freshener in an aerosol dispensed can was observed on top of the file cabinet in space used by children.
