Findings: It was reported that a 2 year old child was restrained in a booster seat on the floor in the classroom during circle time. There was no report of injury to the child, but no other children were observed to have been restrained. The child was observed to be buckled in the booster seat on at least 2 occasions, but in the seat often. Upon inspection, the staff admitted to using a "bumbo" seat on the floor in room A. The chair was observed. The staff explained that there are a couple of children who occasionally use the bumbo chair. The director added that she has asked staff to buckle the children in the bumbo chair if the chair is used, because the bumbo chair is equipped with a strap and buckle. It was also reported that sometimes children choose to use the bumbo chair. The chair was not used during the inspection. After discussion the director plans to remove the bumbo chair from the room immediately. One staff reported that the chair is used to keep the child in place during circle time. Another staff reported that it was used to help children listen to the lesson and to have the child be more of a part of the circle time and to be engaged. Continued in the "remarks" section.

Findings: The director and staff were reminded that the fleece blankets in several cribs should immediately be removed and not used with the infants or toddlers. No soft bedding should be used with resting/napping infants. The OCC recommends that no bedding be in the cribs at all. The room was warm today. The infants/toddlers could use a sleep sack instead of a blanket if a parent believes their child needs that. The blankets were removed from the cribs during the inspection. The non-compliance was corrected on site.

Findings: Staffing patterns that are posted at the Center and those that were given to the Licensing Specialist are inaccurate. They list classrooms as not being open until 7:30am when staff report arriving at 6:15am and caring for children at that time. It also does not list accurate end times as staff report getting school children off of the bus and caring for them until the end of the day which is approximately 5pm. They also do not list the accurate ages as no school age children are listed. Please post and provide accurate staffing patterns in the Center and to OCC.

Findings: The Center's license does not approve the Center to care for children five years and older (except for the variance approved for the Owner's own child who is six years old). During the inspection, Licensing Specialists observed a child being put on the bus by an Aide. More than one staff member reported that they put children on the bus in the morning and receive children off of the bus in the afternoon and keep them until approximately 5pm. They are reportedly up to 10 years old. Center's Owner is requesting an increase in capacity and ability to have school-agers.

Findings: During inspection, Owner was questioned about the staff members observed working in the toddler room. Both were unknown to OCC as current employees. Owner reported that the Lead Teacher and the Aide, who are regularly in the classroom, had the day off. BH and HK were the staff in the room. HK was previously employed there, but had been deleted from the personnel list in October of 2020. Owner reported that she was re-hired on 02/04/2021, but did not notify OCC of this re-hire. BH was hired on 02/08/2021, according to the Owner, but this is the first OCC had heard of her being employed there. Centers have 5 working days in which to notify OCC of an addition of any employee. Please forward their necessary paperwork, to include a personnel list (1203 form), release of information and medical form for each of these employees.
